Medical Records Requests:

For Offices Requesting Records:

If you are requesting medical records for a patient, please fax a completed Authorization to Release Information form to our Medical Records Department at (704) 547-3150.

For Patients, Parents, or Guardians:

• Release Medical Records to yourself - If you are requesting medical records for yourself, please complete and submit a Release of Information form.

  • Please note that requests may be denied in accordance with office policy.​

  • $10 record release fee is due in advance.

• If you are requesting to release medical records out to an office, please complete and submit a Release of Information form.

Disability Claims: 

CEH does not assist with any type of disability claims. We can provide medical records upon receipt of a completed request and proper authorization.
